Arises from the middle portion of the fibula on the anterior surface and the interosseous membrane
The Extensor hallucis longus (Extensor proprius hallucis) is a thin muscle, situated between the Tibialis anterior and the Extensor digitorum longus.
Extensor ossis metatarsi hallucis, a small muscle, sometimes found as a slip from the Extensor hallucis longus, or from the Tibialis anterior, or from the Extensor digitorum longus, or as a distinct muscle; it traverses the same compartment of the transverse ligament with the Extensor hallucis longus.

Clinically these patients all demonstrated an area of maximal tenderness where the first branch of the lateral plantar nerve (nerve to the abductor digiti quinti) is compressed between the taut deep fascia of the abductor hallucis muscle and the medial caudal margin of the quadratus plantae muscle.
After reflecting the muscle superiorly, approximately 5 mm of the interface between the deep fascia of the abductor hallucis and the plantar fascia is also released.
The diagnosis is done on a clinical basis, noting maximal tenderness in the area where the nerve is compressed between the taut deep fascia of the abductor hallucis muscle and the medial caudal margin of the quadratus plantae muscle [3,6].
