He started his career failing to break through at Bayern Munich, and eventually ended back in Denmark with Odense BK. He was then brought to FC København before moving to main rivals Brøndby IF in 1995. This move secured Brøndby a large transfer fee when, after only a season at the club, he moved to England to play for Premier League club Tottenham Hotspur, where he played nearly 100 matches before he was loaned out to Wolverhampton Wanderers for a short period of time in 2000. He moved one league down in 2000 to play for Watford FC, where he stayed for three seasons, after which he returned to Denmark for the Danish Superliga relegation battlers Herfølge Boldklub. Following a bad first half of the 2003-04 season, the coach was fired and Nielsen was promoted to player/coach, in a coaching partnership with former national team player and Esbjerg native Michael Schjønberg, and they managed to finish just above the relegation zone at the end of the season. For the next 2004-05 season, Nielsen decided to focus exclusively on coaching, but to no avail as Herfølge was relegated, and he announced his coaching days were over.
