Bernadette O'Brien was a seventeen year old Irish girl who died at a Smashing Pumpkins' concert. The concert was held in Dublin, and during an energetic version of 'Bullet with Butterfly Wings' a surge from the crowd crushed her against the front barrier. She later died in hospital. The band had repeatedly requested the moshing to stop, but continued to play. The band sent flowers to her funeral, and cancelled their next show as a mark of respect.
