CYP2C19, a member of the cytochrome P-450 enzyme superfamily(catalyzing phase I drug metabolism) [6] affects the metabolism of a number of clinically important drugs, such as proton pump inhibitors (omeprazole [7], lanzoprazole, rabeprazole), tricyclic antidepressants (imipramine, amitryptiline), phenytoin, propranolol and benzodiazepines (diazepam) [8].
This is the first study from the country to establish the CYP2C19 phenotype frequencies in the Gujrati and Marwadi (Western Indian) population.
The clinical implications of the CYP2C19 metabolism are many and varied, ranging from marked efficacy in the treatment of Helicobacter pylori infection with omeprazole in PMs to increased risk of failure of anti-malarial prophylaxis with proguanil in the same subgroup.

We studied the in vivo effect of CYP2C19 substrate (citalopram, omepratzole, or lansopratzole) on the metabolism of endogenous and exogenous melatonin by measuring the excretion of urinary aMT6S, the main metabolite of melatonin, and a reliable estimate of plasma melatonin in 15 insomniac psychogeriatric inpatients.
In conclusion, it may be inferred that CYP2C19 substrate slows the metabolism of exogenous melatonin and increases its bioavailability, as shown by the augmented excretion of aMT6S, probably by inhibiting the conversion of melatonin to NAS via CYP2C19 isozyme.
