Plot outline

Spoiler warning: Plot and/or ending details follow.

The episode opens with Frasier Crane talking on air to a caller, Marco, who tells Frasier that he is only staying with his current girlfriend, Catherine, until someone better comes along. An angry Frasier advises him to break up with his girlfriend immediately. A few days later Roz Doyle tells Frasier that he has a fan outside who wants his autograph. This fan happens to be Catherine. She tells Frasier that Marco broke up with her, and furiously asks why he ruined their relationship. Frasier informs her that he only told Marco to break up with her because he was not committed to their relationship. She begins to cry, and Fraiser consoles her to the point that they realize they are attracted to each other. The next scene opens in Cafe Nervosa, with Frasier and Catherine. They are obviously intimately involved and are talking about dinner plans. Catherine has to go, and as she is leaving Niles Crane spots them through the window kissing. As she departs he enters, and immediately asks Frasier about her. When Frasier reveals to Niles who she is and how he met her, Niles cannot believe Frasier would compromise his ethics by dating someone related to a patient. Frasier dismisses his concerns by claiming Marco was not a patient, he was a caller, and that there is a difference. Back in the studio the next day, Marco calls in and against his will Frasier talks to him. Marco, who obviously cares more for Catherine that he previously realized, states that he saw her with another guy, who he couldn't see clearly, and asks Frasier if he should ask her to get back together with him. Frasier, obviously drawing from his desire to stay with Catherine, tells Marco that he should stop thinking about Catherine and move on with his life. Both Niles, who was listening to the show in his car, and Roz are shocked that Frasier would give this conflicted advice.

Niles picks up Frasier in his car, and reveals that he was listening to the show and asks him if he can honestly say that the advice that he gave Marco was in his best interest. Frasier tells him that he doesn't care if it was or not; he is in love with Catherine. Niles asks Frasier if his stomach has gotten upset, and reminds him that every time Frasier has crossed his ethics in the past he has gotten an upset stomach.

The next night Frasier and Catherine are together at his apartment. After dinner they try to have sex several times, but each time Frasier begins to feel sick to his stomach in the middle. He reveals to Catherine that every time they kiss he begins to feel sick because of her connection with one of his patients, Marco. Catherine cannot believe that he is breaking up with her because of his ethics and storms out of his apartment. Frasier is upset but, as according to Catherine, at least he has his ethics to console him.

Spoilers end here.
