The Pear of Anguish is a torture instrument shaped like a pear, which was inserted in the victim's mouth (hence also called Chokepear), rectum or vagina.
Upon insertion, the handle was turned, causing the leaves to slowly separate causing extreme pain and mutilation to the victim.
The insertion of the pear was dependent (as a mirror punishment) on the crime the victim was accused of; while slanderers and blasphemers would have their mouths mutilated, homosexuals would suffer the same fate on their anuses.
