Chyle is a milky fluid (bodily fluid ) consisting of lymph and emulsified fats , or free fatty acids (FFAs). It is formed in the small intestine during digestion of ingested fatty foods and taken up by lymph vessels (lacteals ), unlike other components of digested food that are conveyed by veins . Technically it is just ordinary lymph fluid that is rich in digested nutrients, free fatty (FF) acids. The lacteals are much better suited for the transport of FF acids away from the gastrointestinal tract. Chyle fistula is defined as a leakage of lymphatic fluid from the lymphatic vessels, typically accumulating in the thoracic or abdominal cavities, but occasionally manifesting as an external fistula.
Chyle fistulas also can form as a result of venous hypertension, and they have been described in patients with superior vena cava syndrome or thrombosis of the vena cava, among others.
For chyle fistulas to form, either a scarcity of lymphatic collaterals must be present or the injury to the lymphatic channels must overwhelm the remaining lymphatic vessels.
