Fisher's finest playoff hour came in Game 5 (May 13, 2004) of the 2003-04 Western Conference semi-finals between the Lakers and the defending champion San Antonio Spurs. With the series 2-2, Game 5 was a tight affair though the Lakers led throughout the game. With 11 seconds remaining, Kobe Bryant hit a jump shot to put the Lakers up 72-71. The Spurs, needing last-minute heroics, thought they had clinched the game when Tim Duncan made an 18-foot shot despite falling away from the basket. The Spurs led 73-72 with 0.4 seconds on the clock. After three consecutive time-outs, Gary Payton found Fisher, who managed to catch, turn, and shoot to hit the game winning shot. Fisher sprinted off the court because, he later admitted, he himself was not sure he had beat the buzzer and wanted to get off the floor before the play could be reviewed. The Spurs immediately filed a dispute regarding the shot anyway and, after reviewing video footage of the play, the referees concluded that the ball had indeed left Fisher's hand before the clock expired. The "0.4" basket counted and the Lakers had won. Fisher was nicknamed "The Fish that Saved L.A" for the play and was credited with saving the championship run, as losing the game might have extended the series to seven games. The Lakers went on to close out the series in Game 6 to defeat the Spurs. After the 2004 season, Fisher became a free agent. Although Fisher was popular, the Lakers saw him as a role player. With Gary Payton and Karl Malone also on the team, Fisher was also removed from the starters list to the bench and playing 18-20 minutes a game. Fisher's two-season term with Golden State proved to be something of a disappointment, as some of the weaknesses in his game were exposed. While he is a good spot-up shooter, without a star player such as Bryant or O'Neal to command a double-team, Fisher saw limited openings, and struggled against quicker players. The team as a whole continued to struggle mightily and languished near the bottom of the Western Conference standings.


Fisher had stated that his primary reason for leaving the Lakers to come to the Warriors was the chance to run his own team as the starting point guard, but Baron Davis soon replaced him in that role and he came off the bench for the majority of his time in Golden State. In the 2005-06 season, he was productive in that back-up role, averaging over 13 points a game, the highest season scoring average of his career. He had stated vaguely a few days before the beginning of the Conference Semifinals that one of his four children was ill, avoiding going into further detail on the serious aspect of it, other than to say he needed to be with his family instead of participating in the game on May 7th.


It was not until after Game two, during an on-court post game interview that Fisher went into more explicit detail on the grave situation involving 10-month-old daughter, Tatum. She had been diagnosed with retinoblastoma, a degenerative and rare form of eye cancer, which required an emergency three-hour surgery and chemotherapy at New York's Presbyterian Hospital. Earlier Derek had asked coach Jerry Sloan to leave him on the active list, but said he could not guarantee he would make it back to play. After receiving permission from the doctors in New York he and his family flew back to their home in Salt Lake City, Utah. It was not until he landed that he found out teammate and starting point Deron Williams was in foul trouble and his backup for the game, Dee Brown, had been seriously injured. He knew his team needed him, as forward Andrei Kirilenko was playing point guard. Derek Fisher arrived at the arena in the middle of the third quarter, suited up, and with no time for warm-ups or a shoot-around, he made his way to the court late in the quarter. As he walked down the bench he received high-fives from teammates, and before he could take a seat on the bench he was called upon by head coach Jerry Sloan to enter the game. Derek has since come out and said he was feeling very nervous and was telling himself to go out there and not do anything. He told himself not to shoot, but just go out there and hold the ball. He entered the game for fill-in point guard Andrei Kirilenko. He received a standing ovation from the crowd, hugs from teammates and a hug from former teammate Baron Davis. He followed through on his word, and did pretty much nothing in the little time he got at the end of the third quarter. The Jazz looked like the game was slipping away in the fourth quarter but started to make a comeback. With little time left in the quarter he made a critical defense stop on the Warrior's star player Baron Davis, which gave the Jazz a chance. This tied up the game and it was sent into overtime. The Jazz had taken a 120-117 lead when Deron Williams found a wideopen Fisher in the corner for three, and Fisher did not hesitate this time. He fired and it went in, sealing the win for the Jazz.


Second run with Lakers

On July 2, 2007, Fisher announced at a news conference with team owner Larry Miller that he was leaving the Jazz to devote his energies to fighting his daughter's illness. He said Utah simply did not have the medical care required for his daughter, and that only 6 cities did. He did not know whether or not he would play basketball again, his main concern was finding the right care for Tatum. However, after much speculation, on July 19, 2007, he officially rejoined the Los Angeles Lakers by signing a 3-year contract worth roughly $14 million.
