The story follows Bob (Dillon) and his "family" of drug addicts as they travel across the Pacific Northwest in the early 1970s, supporting their habit by robbing pharmacies and hospitals. A highlight of the film is an appearance by the original "junky" William S. Burroughs as Tom, a defrocked priest who lectures Bob on the dangers of temptation. After a tragedy strikes the "family" Bob decides to try to "go straight" but finds that there is more to extricating himself from the drug user's lifestyle than just giving up drugs. In the beginning of "Drugstore Cowboy," cleaned-up dope fiend, Bob (Matt Dillon) is being ambulanced away after taking a couple of slugs and a rather brutal shellacking from a pair of vengeful dealers.
Bonnie takes it upon herself to devirginize the sex-shy Clyde, and Marla in "Natural Born Killers" is every bit the death-dealing equal of her homicidal counterpart, and she demonstrates in the first ten minutes that she doesn't think twice about shattering the chattering teeth and breaking the knocking knees of whoever crosses her.
Bob, in "Drugstore Cowboy," pinned down and brutalized by the fuzz, drops his gaze down to the level of a candy cane tie of one of the officers, realizing that this man, too, has a noose about his neck.
