ECF volume depletion is sensed by pressure receptors located in the cardiac atria and thoracic veins and results in increased renal Na conservation.
ECF volume depletion should be suspected in patients with a history of inadequate fluid intake (especially in comatose or disoriented patients); vomiting; diarrhea (or iatrogenic GI losses, eg, nasogastric suction, ileostomy, or colostomy); diuretic therapy; symptoms of diabetes mellitus; and renal or adrenal disease.
When ECF volume depletion is due to or complicated by metabolic disturbances such as diabetic ketoacidosis or Addison's disease, attention should be given to correcting these problems in addition to replacing volume.
