The song is about a man and a woman who once loved each other, and the woman likely left the man for another one. Out of anger, he murders her.

She said to me
I'll always stay with you
but it only seemed
that she played for me alone

The woman promised to be with the man forever. Her playing for him likely represents the loyalty the man thought she had towards him. "It only seemed that she played for me alone" can be interpreted to mean that she was "playing" for somebody else.

I poured her blood
into the fire of my rage
I locked the door
they asked for her

The man murders her and puts her body in the piano room. He locks the door to the piano room. Her parents ask for her.

The door is open
oh, how they scream
I hear the mother pleading
the father is beating me
they take her from the piano
and no one believes me here
that I am deathly ill
from sorrow and the stench

When the door is finally unlocked, the woman's parents see what he has done. He is ill from the stench of the dead body and the sorrow of losing his love.
