The Miss Teen USA pageant was created in 1983 as a sister pageant to the Miss USA system. The first pageant was held in Lakeland, Florida in August, 1983 and was won by Ruth Zakarian of New York. The pageant moved to April the following year, and then was held in January the next two years. It was held in July from 1988-1990 and has been held in August since 1991. The most common host city was Biloxi, Mississippi which hosted the pageant from 1990-1994, which caused problems in 1992 when the conclusion of the live broadcast was delayed because of Hurricane Andrew. Other frequent host cities have been South Padre Island, Texas (1997, 2001-2002), Shreveport, Louisiana (1998-2000) and Palm Springs, California (2003-2004, 2006). Preliminaries - The 51 delegates will compete in a preliminary competition one week before the final. There will be three rounds of competition: Swimsuits, evening gowns, and personal interviews.
- Swimsuit Competition: The 51 delegates will be wearing swimsuits provided by the sponsors.
- Evening Gown Competition: The 51 delegates will be wearing an evening gown chosen by themselves.
- Interview Competition: Each of the 51 delegates will be meeting with one of the preliminary judges to speak about a topic.
- The average scores of each delegate will be taken and the top 15 highest average scores will make the top 15. The top 15 semifinalists will then be announced during the final competition.
Final Competition - After the top 15 delegates are announced during the finals, the 15 will compete in the same rounds of competition. However with some differences this time (i.e. The average composite scores of the evening gown and swimsuit competitions were not shown during 2003 to 2006). They include:
- Swimsuit Competition: The 15 delegates will be wearing swimsuits provided by the sponsors.
The average composite scores will be shown on television and the delegates with the highest 10 scores will make the top 10. - Evening Gown Competition: The 10 delegates will be wearing an evening gown chosen by themselves. They can choose to stay with their preliminary gowns or change to a new gown.
The average composite scores will be shown on television. These scores will be added to the average composite scores of the swimsuit competition, and the delegates with the highest 5 scores will make the top 5. - The Final Question: The top 5 delegates will pick out a question purposed by one of the final judges. Each delegate has 30 seconds before they hear a bell ring meaning that time is up.
- Afterwards the final judges will take a final look at the top 5 and cast their votes for who they think should be 4th, 3rd, 2nd, and 1st runners ups and the new Miss Teen USA. Factors taken into consideration are:
- 1. Response to final question (Delegates who answer the question with confidence, substance, poise, vocabulary, and more). Delegates who fail to answer the question might automatically receive 4th runner up (i.e. Miss Missouri USA 2001, Larissa Meek and Miss Venezuela 2005, Monica Spear).
- 2. Average scores from the finals and the preliminaries.
- 3. Overall impression about the delegate.

- After the first five years of the pageant, four states have won the title the first year they placed: Idaho (1989), New Hampshire (1991), Iowa (1992) and Montana (2006). Four states have won the second time they placed: Vermont (1993), Delaware (1999), Wisconsin (2002) and Ohio (2005). Most of these states have also performed poorly in the Miss USA pageant.
- The most successful states are Texas which has placed fifteen times and had one winner and seven runners-up, Georgia which has placed thirteen times and had six runners-up and Tennessee which has had eleven placements, won once and had four runners-up.
- The least successful state is Utah, which has placed only once (2nd runner-up, in 1995). Arkansas, Nebraska and Wyoming have placed twice and have never won the crown. Idaho and Montana have placed once and won the title on that occasion.
- States which have performed well in the Miss Teen USA pageant but poorly in the Miss USA pageant are North Dakota and Washington.
