The novel is about Pope Alexander VI and his family. Puzo spent over twenty years working on the book off and on, while he wrote others. It was finished by his longtime girlfriend, Carol Gino. Effectively his last novel. Pope Alexander VI (1 January 1431 – 18 August 1503), born Roderic Borja (Italian: Borgia), (reigned from 1492 to 1503), is the most controversial of the secular popes of the Renaissance and one whose surname became a byword for the debased standards of the papacy of that era. ...

The book has a factual core, with fictional events used to create the unknown aspects of Pope Alexander VI, formerly Cardinal Rodrigo Borgia, and his family's life. The book is peopled by real characters, including Niccolò Machiavelli, and the real relationships with members of the Borgia family.
