"Business efficiency" here might refer to the ratio of the costs actually paid for an operation, divided by the revenue(or profit?) gained from that business operation.
Unfortunately, having a high rank means that the country has inefficient environment for profit-making operations.

Here in Japan everyone grows up believing that Japan is the most efficient country in the world. Many of my friends were shocked when I showed them how wrong this belief is.

Somehow, I find it hard to believe that Chile is significantly more efficient in its' business than Italy, France, Spain or Greece.
